An Israeli startup behind an app to help patients recover at home following their discharge from the hospital has raised $15 million.
Laguna Health integrates medical care plans with “social, emotional, and cultural factors” to help care managers better support members’ recovery and ongoing care.
Members can communicate with their care managers or experts from the company through the app, phone, or SMS.
The app also makes it easy for care managers to keep track of a patient’s data in one convenient platform.
Published clinical trials using the app have shown to yield 50 percent cost savings and 10 times productivity gains for care managers.
The funding round was led by the venture capital firms SemperVirens and HC9 Ventures.

Some 35 million Americans are hospitalized annually, with at least two-thirds discharged to self-care at home. To recover, these patients typically receive paper instructions and minimal ongoing clinical oversight.
Even if a recovering patient is assigned a care manager, the patient’s medical records rarely reflect their own life context, their caregiver’s life context, or the emotional drivers that may hinder their recovery journey.
